Counselling Contract
Welcome to Cherry Tree Counselling and Psychotherapy Service.
I would first like to thank you for choosing Cherry Tree Counselling and Psychotherapy Service (Cherry Tree CPS).
I am a fully qualified integrative therapeutic counsellor and am a registered member of BACP (British Association of Counsellors and Psychotherapists) and have been Accredited by NCS (National Counselling Society). I am also fully insured and have regular supervision and undertake regular professional development.
Cherry Tree CPS offers open-time contracts, which mean that we will both commit to ongoing therapy until you feel the work is complete.
You can contact me by text on: 07771 666061 or email me at louise.willis@cherrytreecps.co.uk for the purposes of cancellation and rearrangement of appointments only. I also offer telephone and video counselling should you not be able to attend, and home visits in extenuating circumstances.
Sessions will last for 50 minutes to 1 hour each.
The agreed cost per session is £50, and this will need to be paid each session unless via EAP or we have agreed otherwise in advance.
If you are unable to attend for any reason, please give me at least 24 hours cancellation notice, without which the a fee of £30 for the missed session will be charged. Please contact me by text on 07771 666061 or e-mail: louise.willis@cherrytreecps.co.uk.
I provide the highest level of confidentiality possible according to the law and the Code of Ethics of BACP and with current General Data Protection Regulations (GDPR) legislation. I am a registered member of Information Commissioner’s Office (ICO).
However, very rarely some lawful situations could result in one of the following disclosures:
- In situations of concern regarding your emotional/mental well being, I would encourage you to self-disclose in order to access further appropriate support. However if you prefer, and with your consent, I could do this on your behalf, e.g. a mental health condition or self-harming behaviour.
- In situations in which a disclosure of the threat of significant harm to the yourself or a third party is made. I will need to break confidentiality, eg. significant risk of suicide, or risk of significant harm to third party (including animals).
- In situations of public interest in respect of which I would have a legal obligation to disclose information without your consent and also would not be able to inform you prior to taking any such action, e.g. certain illegal activities, including and not limited to: dealing, acts of terrorism and child protection issues.
- A Court orders access to your clinical notes.
Depending on the particular circumstances, I would aim to discuss this with you before taking any action wherever possible.
In order to support any medical or physical disability needs you may have, it could be necessary to conduct a risk assessment together.
All BACP registered professional counsellors are required to attend regular supervision to support professional practice. I may be required to use some information to fulfil my ongoing professional training, however all cases are discussed using a pseudonym and as no identifying details are used, your privacy would therefore always be maintained.
I may make brief notes during or at the end of counselling sessions in order to monitor our work, these are always non-identifiable and are kept in a secure location and are destroyed after 7 years, as is the contract, this is in line with my insurance and BACP guidelines.
In the unlikely event of ill health or me being unable to contact you, I may need to ask my supervisor to contact you on my behalf. In this case I will need to share your contact information.
Any documents containing identifiable details are kept in a locked safe at all times. You can access your data at any time, if you wish to do this or remove your agreement to my use of your data at any point please ask in session, or by email, text or phone.
In our first full session you will be asked to acknowledge that you have read, understood and agreed to the terms above, terms of data collection, usage and disposal and that you are currently not undertaking therapy elsewhere.
Cherry Tree CPS will never pass your data onto any other party other than for the exceptional reasons outlined earlier.
You may choose to end our work together at any time, alternatively you may find the work draws to a natural ending. In either case it may be important to have a closing session, as this can form an integral part of the therapeutic process.
In the case of making a complaint, please in the first instance contact myself or alternatively my Supervisor: Lorraine Cookson. Tel: 07809 385374 Email: info@lorrainecookson.co.uk
Occasionally it may be necessary for me to refer you onward to another health care professional, this could be for a number of reasons such as:
- Your needs could be better met elsewhere.
- Situational changes making the contract untenable.
- We don’t seem to have gelled and are therefore not working well together.
I look forward to working with you.
Louise
